Please make sure that you arrive at our office at least 30 minutes prior to your appointment time to allow us sufficient time to process your paperwork. If you forget to bring your completed forms with you, please arrive at least 40 minutes prior to your appointment time.
It is also necessary to bring the following information with you to your appointment:
- Driver’s license or ID card (State issued) – Note: Minors must also show a picture ID (School ID cards are acceptable)
- Insurance cards
- List of current medications being taken
- Name and phone number of your pharmacy
A copy of these documents will be part of your permanent record. If you have insurance changes, please notify our office at least two business days prior to your appointment.
Laboratory Billing – Any services that are sent out to a laboratory will be billed directly from that laboratory. Thank you in advance for your attention to this matter.
We are pleased to announce that as of November 2019 Care for Women is now utilizing text messages for appointment reminders! You must sign a consent form for us to do so!
If we have your cell phone listed as your primary number and you have consented to text messages via your patient portal, Care for Women will text you to remind you of your appointment(s). Patients will receive text message notifications and will no longer receive a phone call to remind you of your upcoming appointment(s).
Please log onto your patient portal and fill out the consent form. Rest assured if you do not sign the consent form Care for Women will continue notifying you through a phone call.

Care for Women’s Waiting Room Policies
We recognize that the wait for a doctor’s appointment can be stressful at times, especially if a patient has medical problems that create discomfort or anxiety, or if unforeseen events cause an unexpected delay. Therefore, in an effort to create a comfortable environment for all patients, we have instituted the following policies.
For the comfort and privacy of all our patients, we respectfully ask that you provide adequate supervision of your children at all times. You will need to bring another adult to supervise them while you are in the exam room. If your children are causing a disruption while you wait for your appointment, you may be asked to reschedule. Thank you for helping us in our efforts to provide a comfortable and peaceful environment for our patients while they wait.
Regarding Food and Drinks:
Please refrain from bringing food or beverages into the lobby area. Your assistance in keeping our lobby clean and comfortable is sincerely appreciated.
